OLYMPIA – The Washington State Department of Health has revoked or suspended licenses, certifications, or registrations of subsequent health care providers in our state. The department also immediately suspended the powers of people banned from practicing in other states.
The Department of Health Systems Quality Assurance works with councils, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, consultants). Information on bringing doctors and paramedics to disciplinary responsibility can be found on the website Website of the Washington Medical Commission (WMC).. Information about health professionals is available on the agency’s website. Click on «Find a healthcare provider license”In the section“ How am I? ” section c Home page of the Ministry of Health (doh.wa.gov). The site contains information on the status of the license of the health care provider, the date of expiration and renewal of their powers, disciplinary sanctions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who believe that a health care provider has acted unprofessionally are also encouraged to call and report their complaint.
Chelan County
In March 2022, the Hearing and Speech Council suspended the work of a speech therapist Tracy A. Bair Hutchins (LL00003176). Hutchins recorded the treatment of patients, but never provided services while working in a rehab facility.
Clalam County
In March 2022, the Minister of Health revoked the license of a home care assistant John Stephen Gale (HM60604379). In 2021, Gail was convicted of assault with a deadly weapon and harassment with death threats in the Clalam County Supreme Court.
Kitsap County
In April 2022, the Minister of Health suspended the license of a professional trainee for health disorders Douglas Paul Brown (CO60956432). Brown misused controlled substances.
Pierce County
In April 2022, the Nursing Commission suspended the nurse’s license Teresa Elizabeth Keith (RN00114854) after she withdrew from the drug monitoring program in which she agreed to participate to avoid potential discipline.
Snohomysh County
In April 2022, the Minister of Health suspended the license of home care assistant Austin Riley McLaughlin (HM60768975) after he was convicted of assault related to domestic violence in the Marysville Municipal Court.
Spokane County
In April 2022, the Minister of Health suspended the license of a registered paramedic David Yu. Kolesnikov (NA60246330). Kolesnikov did not provide the patient with scheduled medication and took personal belongings out of another patient’s room without permission.
In April 2022, the Minister of Health rejected a request for reactivation of a certified paramedic Muriel R. Snakeskin (NC60086077) and suspended its license. Snakeskin failed to comply with a 2015 agreement that required her to receive additional education and pay a fine.
Outside the state
Montana: In March 2022, the Nursing Commission suspended the nurse’s license Kelly Seas Koch (RN00087640) after the Montana Nurses Council revoked the license of Registered Koch Nurse for failure to comply with the relevant opinion regarding safe nursing practices and improper delegated medical care.
